Thank you for the question. I understand that you're concerned about the potential side effects of semaglutide. Let's first ensure we understand what a side effect is. According to the FDA, side effects, commonly known as adverse reactions, are unwanted, undesirable effects that are possibly related to a drug. Side effects can vary from minor problems like a running nose to life-threatening events. So, in the case of semaglutide and tirzepatide, dosage, and duration of medication will have an impact on side effects. Here is a listing of the most common side effects (adverse reactions) for the respective medication reported in the following clinical trials: SUROUNT-1, SURMOUNT-3, STEP-5, STEP TEENS, and SURPASS-2.
Tirzepatide (SMT1) | Tirzepatide (SMT3) | Semaglutide (STP TNS) | Semaglutide (STP5) | Tirzepatide (SRPS2) | Semaglutide (SRPS2) |

As you review this list, please be aware that because a person experiences one or more side effects does not indicate the medication is more or less effective. It is common for patients who receive a placebo to report the same side effects as the treatment (medication) group.
Overall, semaglutide and tirzepatide are very safe medications when used under the supervision of a provider who is experienced working with them. Regular check-ins are essential as they allow for interaction between the patient and provider to discuss progress and if any side effects are occurring. Congratulations on choosing to lose weight and live a healthier life! Weight loss is individualized and varies significantly from person to person. Working with a provider who is an expert in weight loss and obesity-related issues is essential.
It's important to understand that some people respond quickly to treatment (early responders), while others respond after some time (delayed responders). Regardless of your group, weight gain does not happen overnight. So, it's natural that weight loss will take time, too. This variability is normal and part of the process. You're not alone in this journey, and we're here to support you every step of the way.
Two weight loss pathways exist. The first and most established approach is lifestyle changes, which include diet and exercise. Lifestyle changes can result in weight reduction, but it takes longer, and the amount lost is modest at best. SURMOUNT-1 found lifestyle changes alone resulted in a -3.1% weight reduction at 72 weeks versus -15% to -20.9% on tirzepatide. STEP TEENS found lifestyle alone resulted in a 2.7% increase in body weight at 68 weeks versus -17.4% on semaglutide. STEP-5 found that lifestyle alone resulted in a -0.6% weight reduction versus -16.7% at 104 weeks. Kompaniyets et al. (2023) found that only about 1 in 10 patients could achieve a 5% reduction in weight with lifestyle modifications alone.
The second approach is pharmacological treatment via medication prescribed by a medical provider. Of the two options, pharmacological treatment has the best results in achieving meaningful weight loss. By 'meaningful weight loss, we mean a reduction of at least 5% of your initial body weight. GLP-1s can be incredibly effective in weight reduction. The SURMOUNT and STEP clinical trials document the effectiveness of Semaglitude and Tirzepatide in weight reduction.
Remember, the best formula for weight reduction is combining lifestyle changes with pharmacological treatment. The SCALE trial (2013) was the first document on how effective a GLP-1 (Liraglutide) can be when combined with lifestyle changes. The clinical trials SURMOUNT-1 & 3, STEP-5, and STEP TEENS examined Semaglutide or Tirzepatide plus lifestyle changes and found patients experienced significant weight reduction of up to 24%. With the right approach, success is within reach.
However, patients who rely solely on medication and make no changes in lifestyle see little to no meaningful reduction in weight! So, you must do your part to see a change for the better!

Brenda, I have been on Tirzepatide for 20 weeks and lost nearly 40 pounds. I still have 10 pounds to go to meet my goal weight. Is it safe for me to be on Tirzepatide this long?
Congratulations on your impressive weight loss! Losing nearly 40 pounds is a significant achievement, and I am proud of you! You are changing your life for the better. Let’s discuss your question about the safety of being on Tirzepatide for 20 weeks.
Tirzepatide is a GLP-1 and GIP receptor agonist used to manage type 2 diabetes and promote weight loss. Here are some essential points to consider:
- Duration of Treatment:
- You’ve been on Tirzepatide for 20 weeks, which is a substantial period.
- GLP-1 medications like Tirzepatide and Semaglutide are generally safe for long-term use. Still, it is essential to follow the guidance I provide during our one-on-one consultations.
- Safety and Monitoring:
- Regular (weekly check-ins are best) monitoring is crucial during treatment. I will assess your progress, monitor any side effects, and adjust the dosage if needed.
- Common side effects include gastrointestinal symptoms (such as nausea, vomiting, or diarrhea), which tend to improve over time.
- Obesity Specialist Care:
- You mentioned that you’re still 10 pounds away from your goal weight. Consistency, regular check-ins, keeping a food log, and staying active are critical.
- I recommend that my patients stay on GLP-1 (Tirzepatide or Semaglutide) for at least 12 weeks of continuous treatment to achieve meaningful weight loss. In addition, regular check-ins (ideally weekly) to monitor your progress and side effects.
- Once you achieve your weight loss goal, you will join a growing community of my patients who have lost 50 pounds or more! You even get a special YETI cup!
- Clinical Trials:
- The STEP 5 trial with Semaglutide demonstrated weight reduction over 104 weeks, ranging from 5% to 20%.
- The SURMOUNT-1 trial with Tirzepatide demonstrated weight reduction over 72 weeks, ranging from 15% to 21%.
- Both medications had mild to moderate gastrointestinal side effects.
- Individual Variation:
- Weight loss is highly individualized. What works for one person may not be the same for another.
- Be patient with yourself and maintain realistic expectations.
Remember, Obesity is a chronic relapsing disease that requires continuous effort to control. Weight loss is individualized and never guaranteed. One person’s weight loss experience does not guarantee weight loss for anyone else.
Semaglutide and Phentermine are prescription medications that work differently. While weight loss rate and amount vary from person to person, our research shows that patients on Semaglutide lose 17 pounds to every 3 pounds for patients on Phentermine.
This does not mean everyone will have this experience nor is it a guarantee of weight loss.
Semagltude and Tirzepatide are medications that require a prescription. Semaglutide and Tirzepatide are approved by the FDA for weight management and are glucagon-like protein receptor agonists (GLP-1 RA). However, Tirzepatide is a dual GIP/GLP-1 receptor co-agonist.
While both medications are effective in helping patients lose weight, patients on Tirzepatide tend to lose more weight in the given time frame. In two recent studies (SURMOUNT-1 & STEP1) patients lost 17.8% on Tirzepatide versus 12.4% on Semaglutide more than placebo. So, the bottom line is that both medications are effective in helping patients lose weight when combined with diet and exercise. Patients on Tirzepatide tend to lose more weight than patients on Semaglutide.
It is important to remember that weight loss is individualized and never guaranteed.
Weight loss is a unique journey for every individual, and it can be influenced by various factors such as stress, sleep, and hydration. Semaglutide and Tirzepatide are prescription medications that should only be used under the supervision of a medical professional.
